Insurance and Payment Policy

Our office accepts most major insurance plans to help make your care as accessible as possible. However, insurance coverage can vary widely, so we strongly encourage all patients to verify directly with their insurance provider to confirm whether we are considered in-network under their specific plan.  

To support efficient billing and minimize delays, Center for Dermatology & Skin Surgery requires most patients to maintain a valid credit or debit card on file.

Effective January 1, 2026, all patients—except those with traditional Medicare-will be required to keep a card on file. Once insurance has processed a claim, any patient responsibility may be automatically charged up to $250. A statement will be issued for any remaining balance.

Patients who choose not to keep a card on file will be required to provide a $50 deposit at check-in, which will be applied toward their patient responsibility if applicable. Any remaining balance will be billed accordingly.

What to Bring to Your Visit:

To ensure a smooth check-in process, please bring the following to every appointment:

Your current insurance card

A valid government-issued photo ID

A referral (if required by your insurance plan)

Any applicable copay (Debit cards, Credit Cards, Checks, Cash)
